Expansion of the Pratt Pouch to Four Maternity Hospitals in Ecuador and to Peru

In this blog, we interview Humberto Mata, a pioneer in the application of the Pratt Pouch, first in Ecuador and soon in Peru.  Humberto Mata funds the Pratt Pouch through an NGO, Fundación VIHDA. 

1.  How many HIV tests are you doing per year now of pregnant women?  How many positive are you finding (you can also add in the cumulative for all-time). /. 1. ¿Cuántas pruebas de VIH estás realizando actualmente al año en mujeres embarazadas? ¿Cuántas de esas resultan positivas? (También puedes incluir el acumulado total desde el inicio).

Humberto Mata: At the four largest public maternities in Guayaquil, Ecuador, we performed 18,589 HIV tests in 2024 among pregnant women, with 160 reactive cases reported. Overall, since 2007 through 2024 we have performed 434,356 tests, with 3,302 reactive cases during that period, detecting 183 cases per year on average.

En las cuatro maternidades públicas más grandes de Guayaquil en 2024 se realizaron 18.589 pruebas de VIH a mujeres embarazadas, de las cuales 160 resultaron reactivas. En total, entre los años 2007 y 2024, se han realizado pruebas a 434.356 mujeres embarazadas, con un total de 3.302 casos reactivos, o 183 por año promedio.

2a.  When did you expand to four facilities?  /  2.a. ¿Cuándo se expandieron a cuatro centros de salud?

Humberto Mata: The expansion to four maternity hospitals occurred progressively:

• In 2019, we incorporated Maternidad del Guasmo,

• In 2020, the University Hospital and Guasmo Sur Health Center,

• And in 2022, Monte Sinaí Hospital was added.

2.b.  What can you share about the experience of implementing the Pratt Pouches as a system in a facility-- how long does preparation take? How long does training take?  /  2.b. ¿Qué puedes compartir sobre la experiencia de implementar los Pratt pouch como un sistema en una instalación? ¿Cuánto tiempo toma la preparación?

¿Cuánto tiempo toma la capacitación?

Humberto Mata:Institutions have responded positively to the implementation of the Pratt Pouch system, as it helps optimize medication use. Instead of distributing entire bottles—which can be costly and lead to significant waste—a single bottle can be divided into multiple doses, saving both medication and resources.

Furthermore, the system does not require a large initial investment. It only needs adequate physical space and support from health personnel. The minimum training period is three days, though this may vary depending on the number of staff being trained.

Las instituciones han recibido con buenos ojos la implementación del sistema Pratt Pouch, ya que permite racionalizar el uso de medicamentos. En lugar de entregar frascos completos —lo cual representa un alto costo y genera desperdicio—, un solo frasco puede dividirse en múltiples dosis, lo que optimiza recursos y reduce gastos.

Además, el sistema no requiere una gran inversión inicial. Solo se necesita contar con un espacio físico adecuado y la participación del personal de salud. La capacitación mínima dura tres días, aunque el tiempo puede variar según la cantidad de personal que se requiera capacitar.

2c.  What is the reaction of the mothers and families to the Pratt Pouches?  /  2.c. ¿Cuál es la reacción de las madres y las familias ante las pratt pouch ?

Humberto Mata:Most mothers and families of perinatally exposed newborns respond positively to receiving the medication in the Pratt Pouch format, due to the following advantages:

1. Ease of administration: The pouch is easy to tear and makes it simple to give the medicine to the baby.

2. Accurate dosing: The exact dose is provided, eliminating the need for mothers to measure the medication themselves or handle bottles, and reducing the risk of under- or overdosing.

3. Timely delivery: The pouches are prepared in just one day, and the full treatment is customized based on the newborn’s weight.

En su mayoría, las madres y familiares de los recién nacidos expuestos al VIH reciben de manera positiva la entrega del medicamento en presentación Pratt Pouch, debido a los siguientes beneficios:

1. Facilidad de administración: El sobre es fácil de rasgar y permite administrar la dosis al bebé de forma práctica.

2. Dosificación exacta: Se entrega la cantidad precisa, lo que evita que las madres tengan que medir el medicamento manualmente o manipular frascos, reduciendo el riesgo de errores por subdosificación o sobredosificación.

3. Rapidez en la entrega: Los pouches se preparan en un solo día y el tratamiento completo se ajusta de acuerdo al peso del recién nacido.

3.  Can you share anything about Peru and the initiation of the Pratt Pouch there, at this point?  /  3. ¿Puedes compartir algo sobre Perú en este momento?

Humberto Mata: The meetings regarding the implementation of the Pratt Pouch in Lima, Peru are progressing positively. We have already submitted a draft agreement to the Ministry of Health of Peru, and things are moving in the right direction. This Monday 19^th May 2025 we will presenting in person the last documentation requested, including letters from Maternova and Dr. Malkin at Duke about the Pratt Pouch. We hope to start our pilot project in Lima’s largest public maternity by this summer (2025)

Las reuniones sobre la implementación del Pratt Pouch en Lima, Perú, avanzan positivamente. Ya presentamos un borrador de acuerdo al Ministerio de Salud del Perú, y el proceso avanza por buen camino. Este lunes 19 de mayo de 2025, presentaremos en persona la última documentación solicitada, incluyendo cartas de Maternova y del Dr. Malkin de Duke sobre la bolsa Pratt. Esperamos iniciar nuestro proyecto piloto en la maternidad pública más grande de Lima este verano.

4.  Can you make any recommendations to countries or programs considering the pouches?  /  4. ¿Puedes hacer alguna recomendación a países o programas que estén considerando usar el Pratt pouch ?

Humberto Mata:Based on our experience working with perinatally exposed newborns in the hospitals we support, we strongly recommend the implementation of the Pratt Pouch program. This strategy provides significant benefits for both patients and healthcare institutions.

The pouches are practical, easy to store, properly labeled, and contain accurate doses tailored to each newborn’s weight. This reduces the likelihood of dosing errors and enhances treatment adherence. In addition, newborns have shown good tolerance to the medication in this format.

Implementing this strategy is a key step toward ensuring comprehensive, high-quality care for our youngest patients.
